Student Athlete Physicals

Extracurricular activities can take a physical toll on a student’s body — from the musculoskeletal system to injuries that can affect virtually all parts of the body. An annual student physical is one of the easiest and most effective ways to ensure a student is physically fit and ready to start off the season safely. 

Onsite School Physicals

If Houston Methodist will be performing student physicals onsite at your child’s school, please complete, print and sign the Houston Methodist consent form and the appropriate medical history form. Please have your child bring both forms to their physical.
