Houston Methodist Hospital's Anesthesia Preoperative Evaluation Clinic (APEC) looks forward to providing you with safe, effective preoperative care. Our mission is to make your treatment at Houston Methodist as comfortable and safe as possible.

In some cases, existing medical conditions may impact anesthesia. Not addressing a pre-existing medical condition until just before a surgery will cause a delay in - and possibly a cancellation of - your surgery. For your safety and convenience, you will be evaluated by our Anesthesia Preoperative Evaluation Clinic (APEC) three to five business days before your surgery.

For your convenience, younger, healthier patients with few existing medical conditions may be able to be registered and screened over the phone. Patients with more serious existing medical conditions must be seen in APEC in advance. These patients may be asked to provide a letter of consultation from their internal medicine physician/cardiologist; this report can be faxed to APEC. For patients with cardiac stents or implanted defibrillators, an evaluation by the patient's cardiologist is typically required before surgery can proceed.

The APEC pre-operative evaluation is performed in under an hour and there is no charge for the evaluation itself. Charges, however, will result from any laboratories or tests that are required.

How to Set Up Your APEC Pre-Operative Evaluation in the Medical Center


Before your procedure

Once your doctor’s office has scheduled you for your procedure at Houston Methodist Hospital (on Fannin Street), please begin the insurance/financial and anesthesia clearance process. Before you begin, please gather all of your information: Previous surgeries, physicians and their contact information, insurance information, list of medications, including over-the-counter medications, herbals and other supplements. You will need it when filling out the questionnaire, and it will help if it is readily accessible. Once that has been done, please proceed by doing one of the following: Expedite the process by completing the online form.

The form should not be completed if you are having your procedure performed at any location other than the medical center. If you complete the form online, both Patient Access and the Pre-Operative Evaluation Clinic (APEC) will receive the information required from your form. Your privacy is important to us, so we have ensured your personal health information is sent through a secure online service. Once you’ve submitted your form, you will receive a follow up call from APEC within 3 business days.

(NOTE: If you choose to fill out the print version, you will have to fill out two separate forms for Patient Access and the Anesthesia Pre-Operative Evaluation Clinic. Once you’ve faxed your forms, you will receive a follow up call from the APEC within 3 business days.

Register in person with Patient Access at minimum 3 days prior to your procedure. The Registration desk is located on the first floor of the Houston Methodist Outpatient Center. To expedite the process, you can download the forms above, print and fill them out, then bring them with you.

NOTE: You may need to visit the Anesthesia Pre-Operative Evaluation Clinic (APEC) for routine additional testing. We will inform you during our follow up call or when you register in person whether or not this is needed.

For your health and safety, please remember to stop taking your vitamins, herbals, aspirin and anti-inflammatories if your surgeon has instructed you to.

Advanced Directives

For your convenience, please visit the Advanced Directives website prior to your scheduled procedure: Advanced Directives webpage. Once you’ve visited the page, please download a Medical Power of Attorney form and bring the completed form with you to your next appointment.

EMMI Patient Education

Prior to your procedure, you will be emailed a patient education link from “EMMI”. If you do not receive it within 24 hours of calling or visiting the Pre-Operative Evaluation Clinic (APEC), please call 713.441.5035 for assistance. EMMI consists of short online videos that provide education not only about your hospital stay, but also your anesthesia and procedure. EMMI may help answer many of the questions you and your family may have and ease your worries concerning the procedure.

The morning of your procedure


  • Bring a signed copy of the Directive to Physician and/or Medical Power of Attorney.
  • Wear loose-fitting clothes and leave valuables at home.
  • Remove all your jewelry including your piercings.
  • Please do not wear contact lenses. Wear glasses or bring your contact storage case and solution.
  • Bring a list of your current medications and over the counter supplements including doses and how often you take each one.
  • Do not eat (including gum or mints) or drink after midnight, except a sip of water for taking medications.
  • Dentures, partials and hearing aids need to be removed prior to your surgery. Please bring your cases to store them in.
  • Shower or bathe with antimicrobial (CHG) soap as directed. For instructions, please download the instructions below:
  • Bring your CPAP or BIPAP mask.


When you arrive at the hospital


  • You will be greeted by our staff and taken to a patient room.
  • You may be asked to change into a gown and an IV may be placed in your arm.
  • Before your procedure, you will speak with your doctor, an anesthesiologist or CRNA and the nurse who will be present for your procedure.
  • Your procedure start time is an estimate. If we anticipate any delays, we will keep you updated.


During your procedure


  • The room may be cool. Warm blankets are available at your request.
  • Monitoring equipment such as a blood pressure cuff and EKG leads will be placed.
  • Your nurse will remain present during your procedure to ensure that your needs are met.
  • Your family or friends will be in the family room.
  • Your doctor will notify them when your procedure is over.


After your procedure


  • You will be taken to recovery where a nurse will care for you for approximately one to two hours.
  • You will remain on monitors at this time and the nurse will prepare you for transfer back to a patient room.
  • You may briefly have a visitor while in recovery. The family room liaison will bring in visitors.
  • If you are an Outpatient, make sure a responsible adult is available to accompany you home on the day of your procedure. We recommend that they stay with you for 24 hours after your procedure.

From Southwest: Take 59 North to Highway 288 South. Exit MacGregor and turn right. Continue until the Fannin Street signal and turn left. Houston Methodist Hospital is located on the left at 6565 Fannin Street.
