HMCC PROGRAM COLLABORATIONS 

  • Houston Methodist Coordinated Care (HMCC) strives to improve health outcomes for the patients we manage, provide an enhanced patient care experience, and reduce cost through our CVS ACO collaboration. This performance-based collaboration, which includes joint accountability, will focus on helping improve our clinical quality measures through targeted clinical messaging where CVS ACO will deliver information to encourage patients to schedule breast and colon cancer screenings. The program was approved by the HMCC Governance Board in March 2018.  
  • Houston Methodist Primary Care Group collaborates with Next Level Urgent Care for the provision of after-hours care for its patients in an effort to allow patients greater access to care at night and on the weekends without over-utilizing emergency departments when their primary care physician offices are closed.
