Azelastine May Reduce Viral and Bacterial Infections: New Research Signals Shift in Sinus Care

Azelastine, a common nasal antihistamine, may reduce viral and bacterial infections while helping prevent chronic sinus disease — highlighting a shift toward predictive and preventive rhinology.

For Dr. Omar Ahmed, an associate professor of otolaryngology, the momentum around his recent research is both exciting and validating. His latest study reflects a growing shift in rhinology: moving upstream from treating chronic sinus disease to predicting and preventing it.

The project published in the International Forum of Allergy & Rhinology (IFAR), builds on a simple but compelling observation. A commonly prescribed intranasal antihistamine, azelastine, may do far more than relieve allergy symptoms. According to Dr. Ahmed, emerging evidence suggests it may also reduce viral uptake in the nasal mucosa, an insight that could reshape how clinicians think about early intervention.

From allergy relief to antiviral potential

The idea originated from laboratory findings showing decreased viral uptake in nasal epithelial cells exposed to azelastine.

“Something about this medication seemed to reduce the ability of viruses to enter the mucosa,” Dr. Ahmed explains. “We’re still learning why. There may be a connection to insulin growth factor receptors, but the signal was strong enough to explore clinically.”

That exploration led to a retrospective review spanning two institutions — Houston Methodist and a collaborating site in West Virginia. The geographic diversity, he notes, strengthens the findings by capturing different allergen profiles and patient populations.

The team, which included Dr. Masayoshi Takashima, who was a senior author on this paper, found a decrease in acute bacterial infections in patients with recurrent acute sinusitis. Even more striking was a 60% decrease in bacterial sinus infections, a meaningful outcome for patients who cycle through recurrent sinusitis multiple times per year.

A preventive tool for high‑risk patients

For Dr. Ahmed, the implications extend beyond symptom management. His team is simultaneously advancing predictive modeling to identify patients at risk for early or progressive sinus disease. The convergence of these efforts — risk prediction and a potential preventive therapy — could open a new chapter in sinus care.

“Imagine being able to tell a patient, based on our model, that they’re likely to develop recurrent infections. Now imagine pairing that with a medication that may actually reduce chronic sinusitis before they escalate. That’s where this becomes powerful.”

Patients who experience more than four infections per year often progress to surgical intervention. A safe, accessible therapy that reduces the frequency of infections could delay or even prevent that trajectory.

A broader research pipeline

The study's impact doesn't end there. Dr. Ahmed continues to build a robust research portfolio. His team recently completed a study using predictive modeling to identify early-stage chronic sinusitis — another project gaining recognition for its innovation and clinical relevance.

“We have a lot of exciting work happening,” he says.

The current study draws on data dating back to 2020, offering a multi‑year view of patient outcomes. Now, Dr. Ahmed remains focused on refining the science and ensuring the findings translate meaningfully into clinical practice.

“This is remarkable,” he says. “We may be looking at a preventive medication that helps us intervene earlier, reduce infections and ultimately change the course of disease.”
