Intake Forms

Your First Appointment

  • We ask that you arrive 30 minutes before your first appointment, to complete all necessary paperwork. Please bring a list of any medications you are taking, copies of prescriptions, and any other related diagnostic test reports, such a MRI or X-ray scans that you may have available. Arriving early will enable you to complete patient information forms without missing any valuable time with the therapist.
  • Wear comfortable clothing, such as shorts and loose-fitting shirts. Please, no skirts or dresses. Depending on your therapy, you may be asked to lie down or sit on a table; be sure you can do so in the clothing you wear.
  • Your therapist will conduct an evaluation during your first appointment. You will be asked a series of questions, and measurements will be taken to identify your limitations related to self-care, household tasks, leisure activities, and job duties, especially as these relate to areas of pain, stiffness, or weakness. This evaluation is used to develop a therapy plan to improve your ability to perform the activities that are important and meaningful to you in your day-to-day life.
  • Following the evaluation, your therapist will discuss the findings with you and discuss what you will work on during future sessions. You may also learn about exercises you can perform at home that may help accelerate your recovery.
  • It is not uncommon for patients to experience soreness after their initial evaluation. However, if you experience more pain and soreness than usual following treatment, please inform your therapist as your treatment plan may need to be modified.

About Your Sessions

  • Therapy sessions generally take 45 minutes to one hour.
  • To accommodate all patients and increase the consistency of your care, therapists may treat more than one patient simultaneously. In addition to your evaluating therapist, you will be attended by therapy assistants and technicians.
  • A re-evaluation will be completed at least every 30 days to assess your progress. This is often required by your physician and insurance company.  It is also required by the Executive Council of Physical Therapy and Occupational Therapy Examiners for patients receiving physical therapy.
  • Houston Methodist Sugar Land Hospital and Methodist Orthopaedic Specialists of Texas provide translators in a wide range of languages. If you or a family member needs translation assistance, please advise the front desk prior to your therapy.

Attendance and Appointments

  • Regular attendance is very important in achieving your therapy goals.
  • If you do need to change your appointment, please do so at least 48 hours in advance.
  • Please note that the end of the day is our busiest time. If you require late afternoon or evening appointments, please schedule well in advance.

Aquatic Therapy

  • If you will be receiving aquatic therapy, please bring a towel and water shoes to your appointment. Water shoes provide increased comfort and safety in and around the pool area.
  • Our pool water is warmed to 95 degrees for pain management and arthritis.
