San Jacinto Methodist Hospital Partners with Legacy Community Health ServicesBaytown, TX - 6/27/2013
San Jacinto Methodist Hospital announced today a new partnership with Legacy Community Health Services, that expands outpatient primary care in Baytown and surrounding communities.
Legacy has acquired the Family Medicine Center located on the 4th floor of Independence Plaza II, adjacent to San Jacinto Methodist Hospital, and will begin seeing patients in this new location on July 1. Through this collaborative effort, quality primary care will be readily available to more of the Baytown community. Family medicine physicians, along with the San Jacinto Methodist Hospital Family Medicine Residents, will continue to provide care on the 4th Floor of Independence Plaza II to patients currently seen in the Family Medicine Center, as well as patients in need of a medical home.
Legacy has been treating patients, regardless of their ability to pay for more than 30 years. In 2012, Legacy provided care to over 60,000 patients in the Southeast Texas region through five main locations, including their current behavioral health clinic located at 6730 Independence Blvd. in Baytown. This effort will allow Legacy to expand their Baytown services to include primary care in the San Jacinto Methodist Hospital location.

Its team of health care professionals tailor services to meet the needs of patients of all ages,” said Donna Gares, CEO of San Jacinto Methodist.
As a full service, Federally Qualified Health Center (FQHC), Legacy identifies unmet needs and gaps in health-related services and develops client-centered programs and comprehensive health care to uninsured and underinsured populations.
“We are excited to partner with such an established member of the community in our effort to expand services in Baytown. Since opening our Behavioral Health Clinic in Baytown last fall, the need for more comprehensive care has become clear to us and this offers the perfect opportunity,” Legacy Executive Director Katy Caldwell said.
Legacy will continue to provide behavioral health services at the Independence location and expects to continue primary care and offer additional services at the San Jacinto Methodist Hospital location over time.
All Legacy campuses accept Medicaid, Medicare and CHIP, in addition to private insurance, and offer a five-tiered, sliding-scale system for patients who are uninsured or underinsured.
