The Skilled Nursing Facility, located on the 8th floor of West Pavilion, houses (25) beds in semi-private and private rooms. The goal of the Skilled Nursing Facility staff is to create a homelike, and safe environment; therefore, patients are encouraged to wear street clothes and are welcome to bring family photographs, books, mementos, and other favorite items that enhance their surroundings.
The major diagnoses of the patient population include: congestive heart failure, diabetes, coronary artery disease, hypertension, renal disease, and chronic obstructive pulmonary disease. Patients who require wound care, medication regulation, tube feedings, medication management and physical therapy/occupational therapy are also served.
Nurses on the Skilled Nursing Facility have the opportunity to interact with patients and become involved in patient and family teaching. An interdisciplinary team approach provides comprehensive, preventive, and restorative health care services, that assist the patient in transitioning from hospital to home care. The team includes Physicians, Nurses, Physical and Occupational Therapists, Recreational Therapies, Speech Therapies, Dietician, Social Worker, Care Managers, Pharmacists, and Pastoral Services.
Weekly interdisciplinary team conferences are held to establish goals, estimate length of stay and develop a discharge plan for each patient. Patients and family are provided an opportunity to attend the team conference and to participate in goal setting. The patient's progress toward achievement of these goals is also reviewed weekly by the team. Established goals are reviewed with the patient and family/significant other by the Social Worker and Care Manager.
